This page contains forms in PDF format that you can download to apply for tests, renewals etc.
Fill them in and send them by mail or Fax (credit card only) to the office with payment as appropriate. The address details are on the forms themselves.
Non Members
Mensa Entry Test - Dates are shown in the menu bar on the left of this screen and details of time and suburb for each state here. If you would like to sit the Mensa admission test, please read the Join Mensa page and download the Supervised Test Request Form (PDF). Cost $ 60 or $ 40 for concession holders.
Home Test - If you would like to try the Mensa home practice test, please download and complete the Home Test Request Form (PDF). Cost $ 40.
Psychologist Form - If you have already had an IQ test with a registered psychologist please read the Join Mensa page and download the Psychologist Form Request (PDF). Cost $ 40.
Members
Annual fees $ 88 (or $ 44 for concession holders)
Rejoin Lapsed Membership - If you were previously a member and wish to rejoin, please download and complete the Lapsed Member Rejoin Form (PDF).
Transfer International Membership - If you are or were a member in another country and wish to join Mensa Australia, please download and complete the International Transfer Form (PDF). If you are financial in the other country, the balance of your subscription period will be honoured in Australia and no payment will be required.
Renew Current Membership - If your Membership is about to lapse or has lapsed in the last 2 months and you wish to renew, please download and complete the Membership Renewal Form (PDF).
Nominations and applications for office in Mensa Australia These are available in the Members only area.
On-line Payments
Mensa offers on-line payment options using a Visa or Mastercard or a PayPal account. These payments are handled by the secure system of PayPal.
Non members can apply for and pay for Home and Supervised tests or Psychologist forms here.
Members can pay for their renewals by logging in to the site and using the on-line payment option in the members only menu.
If you use the on-line payment system, DO NOT download and submit a form as well.
We will also accept applications via email if you are paying by credit card. If you have a scanner you may fill in the form and send the scanned image as a jpg or PDF file attached to an email.
However, you should be aware that sending credit details via email has a potential risk, even when sent in an attachment, and decide for yourself whether that risk is acceptable. We would recommend you to use the on-line payment options described above as they are more secure.
You may consider ways to reduce this risk. For example:
You may wish to send the form with part of the credit card detail filled in and a second email with the other half.
You may zip the jpg or PDF with a password and send the unzip password in a separate email.

If you are sending two emails for credit card security, make sure we can match the two by using the same (or similar) subject for both.
